Privacy Notice
Information we collect
We collect the name, phone number, email address, ZIP code, and general dental-treatment category you voluntarily submit. We do not currently accept dental X-rays, treatment plans, or other medical documents.
Provider applications
Dental practices applying to join the network may submit business contact details, practice location, provider type, treatments offered, website, and related business information. Provider applicants must not submit patient information.
Never submit highly sensitive information
Do not send us a Social Security number, date of birth, insurance/member ID, bank or card information, password, dental X-ray, treatment plan, or other medical information. Do not email dental documents.
How we use and share information
We use your information to respond to your request, evaluate possible matches, communicate with you, prevent misuse, and operate the service. With your consent, we may share only the information reasonably needed for matching with selected participating dental providers. Providers are independent businesses with their own privacy practices. We do not sell your personal information or use it for targeted advertising.
Retention and security
We limit access to submitted information and use reasonable administrative and technical safeguards. No internet service can guarantee absolute security. We retain inquiries only as long as reasonably needed for matching, legal, fraud-prevention, and recordkeeping purposes, then delete or de-identify them when appropriate.
Your choices
You may ask to access, correct, or delete your submitted information, or withdraw permission for future sharing. Some records may be retained when required by law or for legitimate security and recordkeeping needs.
Children
The form is for adults age 18 or older. A parent or legal guardian may submit a request concerning a minor. Children should not submit information themselves.
