Terms of Use
Matching service only
Smile Savings Match is an information and referral service. We are not a dentist, dental practice, insurer, lender, or emergency service. We do not diagnose conditions, recommend treatment, or provide medical or financial advice.
Independent providers
Participating providers are independent and are responsible for their credentials, examinations, diagnoses, treatment, prices, financing terms, privacy practices, and patient care. A match is not an endorsement or guarantee. Verify a provider and all costs before accepting treatment.
Provider applications
Submitting a provider application is free and does not begin a membership or create a payment obligation. An application does not guarantee acceptance, exclusivity, patient inquiries, or a specific number of referrals. Applicants must be authorized to act for the practice and provide accurate business information. Smile Savings Match may verify credentials and decline or remove a provider.
Pilot Practice Offer
The first approved pilot practice may receive a waived $49 activation fee and a 60-day free trial. After the trial, the membership is $99 per month for the first 12 paid months and may be canceled before the next billing date. Smile Savings Match does not charge a percentage of treatment fees. Final membership terms will be provided in a separate provider agreement before activation.
No guaranteed result
We do not guarantee that a provider will accept a request, offer a lower price, approve financing, or produce a particular clinical outcome. Final pricing may change after an in-person examination.
Who may use the form
You must be at least 18 and submit accurate information for yourself or as a patient’s parent or legal guardian. Do not use the service for an emergency.
Prohibited information
Do not submit Social Security numbers, dates of birth, insurance/member IDs, payment information, passwords, dental X-rays, treatment plans, or other medical information. Do not email dental documents.
Acceptable use
Do not submit false information, impersonate another person, interfere with the service, attempt unauthorized access, or use the site for unlawful activity. We may reject or remove requests to protect users and the service.
